For this, please follow the instruction:
– Create or open needed contact Form
– Move to Fields Tab and create 4 fields – Name, Email, Submit Button and Subscribe checkbox
– Subscribe check box mark as required and with default value “yes” – http://prntscr.com/enf68b
– Save changes
– Move to Conditional Logic Tab
– In conditional section choose – Field – Subscribe – Equal – yes
– In actions section choose – Field – Name – Show
– In actions section choose – Field – Email – Show – http://prntscr.com/enf7mz
