Here are the steps how to do it:
1) Download the Contact Form by Supsystic plugin and activate it.
2) Create your first contact form – http://prntscr.com/b8b6n4 and save it.
3) Copy the shortcode of your Contact Form http://prntscr.com/b8b7fj
4) Open Popup by Subsystic plugin, open the settings of the required popup from the list
5) Open Design tab -> Texts tab -> Text editor
6) Insert the shortcode of the contact form into the Text editor – http://prntscr.com/b8b98b
7) Save the popup and open it on frontend of your website.
